Medical Requirements
Georgia law requires that each child who enters school for the first time has eye, ear, and dental examinations. You may go to the County Health Department or to your own doctor to obtain Form 3300 which documents the required dental, vision, and hearing screenings. The child must be immunized against Diphtheria, Whooping Cough, Tetanus, Polio, Mumps, Rubella (red measles), German measles, Hepatitis B, and Varicella (chicken pox). These immunizations must be recorded on Form 3231 and are necessary to enter or attend school in Georgia.
Contagious Diseases
A child with a temperature of over 100 ºF should stay home until he/she is fever free for 24 hours without the intervention of fever reducing medicine. Head lice, strep throat, staph, "pink eye", impetigo, chickenpox and other contagious diseases should be cleared up before returning to school. In some instances, the school may require a written statement from the doctor to readmit the child to school.
In the case of illness or emergency during school hours, the clinic will call the parents or the person listed on the record to come for the child. In case of a serious accident, action can come only at the direction of the parent. Parents are encouraged to sign a release form to be kept in the office in case of an emergency. Additional emergency contacts are essential.

Medicine
Prescription or over-the-counter medicine given at school requires a completed permission form signed by the doctor and a parent. Parents must supply all medications. Permission forms are good for one school year and may be obtained in the office or clinic. Medications are administered in the school clinic and must be in the original packaging or prescription bottle. If a student must carry an inhaler or epi-pen, an additional form must be completed. All other medications must be provided by a parent directly to the clinic. No medications will be provided by the school.
